Video footage shows a crash involving two L Taraval trains near the San Francisco Zoo on July 31, after an operator reportedly fell asleep while driving. SFMTA confirmed that although no injuries were reported, the initial determination for the incident was operator error caused by fatigue. The train operator has been placed on non-driving status while the official investigation continues.
