Workers killed and many treated after refinery chemical release as investigators probe cause

A deadly chemical release at a West Virginia refinery killed two people and hospitalized over a dozen others, including seven EMS workers. The incident at Catalyst Refiners in the Institute area triggered a temporary shelter-in-place order, which has since been lifted.
